Critical Health Violations: Anonymous caller complained employees forced to leave drive-up window up because air conditioner broken in kitchen. Inspector found broken AC and a large amount of flies present. Inspector ordered AC repair and pest control to treat flies.

Critical Health Violations: Live roaches in several areas of kitchen as well as house flies

Other Inspector Observations: No shield on lights in walk-in cooler; broken walk-in cooler door; hand wash sink being used for purpose other than washing hands; dirty ice machine; excessive clutter on shelves

Month: March

Critical Health Violations: Complaint that an employee with Hepatitis C cuts herself while prepping foods. Inspector spotted open bleeding wound on left thumb of employee while she was prepping biscuits. Employee said she wears a glove. Inspector educated management that staff must cover cuts and be gloved at all times while prepping or around foods. Employees needs a blood test to see if she has active Hepatitis C.

Critical violation summary since 2006 (six inspections):

• June 2007, stored foods not dated with discard date; broken backflow device on sink; broken hand wash sink; flies entering through broken back door; dirty high chairs in dining room
• July 2007, numerous house flies; dirty high chairs in dining room; “person-in-charge” not on site at all times
• November 2007, patron complaint that employees not wearing hair restraints and they are chewing tobacco products. Inspector found no such violations. Educated management about rules on tobacco and hair restraints.
• March 2006, customer complained about silverfish in box of chicken. Inspector found no pest problem.
• December 2006, open employee drink in kitchen
